Help with Proof of Medical Exam. How to get Copy 1 of Med Report?

My husband just went for his medical exam today (1 of 2 Panel Physicians in Maryland, USA). The staff were apparently quite confused and asked him for an IME number (which we don't have) then they asked for an IME form (which we also don't have). After a lot of discussion and time he ended up seeing the physician.

My question is - Where do we get Copy 1 of Medical Report (IMM1017)? The secretart/nurse said that everything was electronic. After paying $340 USD we were hoping to leave with some sort of proof that he did his exam. My husband insisted on having some type of proof so they finally printed out some sort of tracking sheet.

I've searched on the forum but I can't find anything about how to go about getting this copy.

We are so close to finishing our application....please help

There isn't a form with the electronic medical system they are using now. The tracking sheet and doctors receipts are all I got with my medical. The tracking sheet has the IME number on it and you just enclose that sheet (after taking a copy for yourself) with the application.

The IMM1017 doesn't apply to electronic medical reports. I hope this eases your mind.

Thanks. The tracking sheet has a UMI barcode and number. Further down on the sheet under Client visa details it says, "IME" but there is no number beside it. It just says "EDE". My husband said that the secretary said that the medical report would be linked to his passport number. It's so worrisome because I don't want any mistakes to be made and the people in the office didn't seem all that knowledgeable about the process so if they make a mistake...

Anyways, we have a receipt and the "emedical tracking sheet".

I hope that is enough.

I looked at mine again and it's the same as your husbands - there is a UMI number but no IME. I was really worried when I did mine because there seemed to be a lot of confusion over the e-Medical procedures but it all worked out okay. CIC got mine okay and I'm sure they will also get your husband's without any problems.

You shouldn't need any forms and it 'should' be attached to your application when the doctor sends it to CIC. To ensure it 'finds' you application what i would do is include a note stating you did your application and note the doctors name, name of the hospital/clinic and the date you took the exam. If you've already submitted your app just email them that information if it isn't included on the tracking sheet you mentioned.

Thank you Catou and DaveSav. If it weren't for your posts and this forum my husband and I would probably spend the next few months worrying about this. I will send in the copy of the tracking sheet, letter stating doctor, date etc as well as the receipt. Phew. Thanks again.
