hello every one need help
we received in 05/08/15 the Medical Examination by email from the CIC
but we just had new baby that yet was on the application so we send to the CIC the new Docs also the Medical form for my wife was with wrong last name (the name Before marriage) they received the email about the wrong form in the 07/09/15
2 days ago we received the medical form for your new baby but they didn't send anything about the wrong form for my wife
now we need to do the medical exams in 30 days
can we take them with the wrong name on the form ? should we wait ? should we call CIC ?
