Help with GCMS notes

I know there are already threads about GCMS notes but I couldnt find the answers to my questions.

1. My husband and I want to apply for GCMS notes and if he is the main applicant on our PR application (we are residing in Canada) he needs to fill out all the details in the online ATIP form as a Requester, right?!

2. Do we need to fill out & attach the form IMM 5744 for us to see my details as his spouse on the notes? If so, who fills out Section A, B & C? Its really confusing.

3. Do we need to attach proof of address?

You can apply GCMS notes online.

You can follow both the link. It is very easy to apply and it is also very helpfull.

No you do not need to feel out any form just follow the procedures which on link i share with you. May be you have to pay $5 for GCMS notes.
