Help required: IMM1295E Application

Hi,

I'm currently on a PGWP in Canada and looking to apply for SOWP for my wife.
I have few questions regarding the main application IMM1295E.

1. Type of work permit - Should it be "Open Work Permit" or "Other"
2. Details of Work in Canada - Duration (Should I give the duration of my work permit) Job Title (Can I leave those empty)
