HELP PLS - Vancouver office lost applicant's test result!

A friend of mine took his test in May 2014, and still have no news for his oath. So he called the CIC center and was told they don't have his test result. And guess what's happened?! Now he was told they don't have his test information and questioned if he went to do the test!

has this happened before? What's the chance that they lost his test paper? And any suggestions what to do?

Update - he was helped by MP to find out Vancouver office don't have his test record.
