help pls experts, I am confused

I will be applying for CEC by July 2012. I worked for 4 different employer since i graduated and quit working for 2 of them after working for like 3 months on call bases( no particular hours worked per week) During my tax this february, i got a T5 slip from one of them(previous employer) and when i took it to the tax doer, i was told that it's like self employment since they never deduct tax from me that's y they issued me T5 instead of T4. I was told to declare the T5 income to the government because a copy have already been sent to them. I then declared the income as i was told and they deducted all the taxes that i wasnt paying.
I also got the required T4's from my other 3 employers. my questions are as follow:
How will i explain the T5 to immigration?? I worked on call for this company and it lasted only for 3 months.
I currently have 2 companies that i am working part time for and i have gained all the required hours(1950) from them both. The T5 company are not willing to provide any letter of refrence for me since I dumped them for other companies. pls advice me on what to do.

I don't see it as an issue provided you had a valid work permit. The 1950hrs came from the other two companies who are willing to provide emplyment letters and you only need to provide the most recent T4s and NOA(based on 2011 income).
For jobs you don't have employment letters, provide paystubs, work contracts/job offer letters, ROE and a letter stating your state and stop dates and why you could not get the letters. You should be fine.

the problem is how to get ROE and paystubs from the T5 company since they are employment agency, they never send my paystubs, i only get paid from them through direct deposit to my account after i send them my work time sheets. I dont have any job offer letter neither do i have any any work contracts with them. I have paystubs from other companies except this company.
All my companies are employment agencies. I don't know if they will issue ROE. pls advice.

Do i attach a copy of the T5 to prove that i have worked with them even though i am not claiming my hours from them?????

The section applicable to you is under "If you are not applying for EI benefits" - you will have to request an access code and with it register for "My service canada account" and then you can view your ROE.

Apparently, an employer must submit it when you stop working for them, even if you don't plan to apply for EI benefits.

Thanx Jes and Senorita. The tax form is called T5018 . They call it Statement of Contract Payments and a copy is usually sent to CRA. I really dont understand why they issued that to me. Can I include it in my application or leave it and include ROE when i get it???? then write a letter and explain

I don't see any problem, either. The hours will not count, but as long as you were authorized to to that work, not a problem. You should write an explanation of why you can't get the letter, attach the T5018, and be done with it.
