My husband is going to do his Medical Clearance today at the only designated/approved medical clinic here in Ulaanbaatar,Mongolia. They told us to bring all our documents with us i.e. passport,5 photos +relevant documents. So, I printed off IMM1017 Medical Report:Section A-EDE,Spouse or Common-Law Partner in Family Class. Is this the correct document + the Regional Specific Form?? I see on IMM1017 that it says to put the contact address /person within Canada but I'm here with him so do we just mark N/A?
I know the clinic is supposed to provide all of these docs but they were useless when I made the appointment and told me that I had to bring them myself!
 
If you are talking about this form: IMM 1017 SCLE (See here: http://www.cic.gc.ca/english/pdf/kits/forms/IMM1017SCLE.pdf) then that is the wrong form! That is for in-Canada sponsorship. You need to print out Appendix C from the country-specific guidelines for Mongolia available here: http://www.cic.gc.ca/english/pdf/kits/guides/3907e.pdf By the way, the DMP is NOT supposed to provide these documents -- it is the applicant's responsibility to obtain them and bring them to the appointment.

The DMP will give you "Copy 2 of the Medical Report: IMM 1017 EFC - Medical Report Section A". This is what you submit with your application. See page 6 of the Immigrant's Guide: http://www.cic.gc.ca/english/pdf/kits/guides/3999E.pdf
 
Ariell,you are a life-saver...I almost brought the wrong form. So, I just have to bring the Appendix C for the Country Specific Guidelines for Mongolia correct? Plus passport and photos?
 
That's it, unless you have a medical condition and need to bring any reports. Glad you got the info. in time! :)
