hello.
can anybody share THAT DO I NEED TO INCLUDE SUPPLEMENTARY INFORMATION FORM in visa application
2) should i include checklist which is on cic website or that which is on canadian embassy in pakistan's website.

No you dont. Instead you'll have to submit the Family Information Form with your application. (CHC Islamabad).
I had my Supplementary Information Form completed with the application but the submission officer handed it back and said it is not needed, they get the required information from the Family Information Form instead.

* I am a working professional and the information desk officer said that for those who are working, Family Information Form is a must and Supplementary Information Form is not necessary. He also added that even if you are not a working professional, Supplementary Infomation Form is not submitted with the application but you must submit the "Family Information Form".

Moreover, TRV Form also has to be submitted along with the application.
