HELP!!New Request from the CIC: Course No. and Address History in Canada!!

I applied PGWP in May 18th, I have submitted the receipt of $100 of the holder fee as the CIC requested in September, and now I just got a new letter from them asking me to submit/upload three new documents:

1. The number of courses that were in-class and the number of courses that were online
2. Graduation
3. Address history in Canada (include full address and dates)

I have two questions about it:

1. I have the final transcript of my program shows all the courses I took but without indicating which ones are in-class or online. Is it okay that I provide this or should I do something else?
2. Are they suggesting that I should make an address list on the WORD document and tranform it into PDF? I don't think there is an official document for my address history in Canada...

Does anyone receive the same request as I did? Please help me out, please!! I am so anxious about whether I am getting this permit or not eventually...I've been waiting it for too long... ??? ???

1. Send what they request, if CIC is making this request, there is a good chance that they know, or suspect that some of your courses are on line courses. So, transcript isn't enough, they want the courses that you attended in person and those that were on-line.
2. Doesn't matter how you do it, as long as you can upload the address list.

--But does it mean that I should make a note/memo beside my transcript saying the number of in-class and online courses I took?
--Also, they requests the dates of my address history, does it mean that I should put my starting date of every address? Does the CIC need a proof of each address? Because I have lived in a home-stay for a year and then lived in my friend's house and there are no proof/receipts for these places...All I need to do is just to type down the address and dates and need no proof at all??

But I could not find any documents specifically indicating which ones are in-class or online...Is it okay just to add a memo on my transcript and type down the number of inclass and online courses??

Just make a list of courses you have done and indicate particularly that which one is done in person and which one is online referring them to transcript and get it stamped and signed by your college/university or ask your college/university to prepare one for you indicating all courses and as a address proof you can prepare a doc stating your duration of stay in that particular place and as a proof you can add older bank statements which you can find in account online otherwise a copy of your older driver's license, anything that shows the address you have lived in (it might be a copy of your study permit).
