Help needed to fill Schedule A/IMM 5669 Background Declaration form

I got a request to submit Schedule A Background Declaration form. I have some questions for that.

What standards are considered as elementary and secondary school ?

What should I write in the table if that question does not apply to me. i.e I never worked for military or with any organisations.

Please reply me fast as I have to submit it.

I can write my name in native language by pen?
