Help Needed Study permit Extension and Medical Exam Inside Canada

Hi, I received the study permit for high and secondary school but now I have been admitted to a DLI listed University so please tell me should I get the study permit extension or i will apply for a new study permit do I need a medical exam too as I am in Canada right now. Need your expert opinion and if I need to apply for a study permit extension which documents should i attach to my application?
