HELP needed in filling Courier and Consent form

Hey! I've just received the forms and pickup is for tomorrow and there are some things I'm not able to understand what should i fill in them:

1. In consent form do i have to fill in my address as the one in my passport? I reside at a different place now. Or do i have to fill my temporary address from where the passport will be picked up?
2. In consent form do i have to fill the section 'Declaration to be signed ONLY by applicants assisted by the CVAC staff with electronic application form.' ?
3. In consent form there are two sections - 'Applicants wishing to provide biometrics' & 'Applicants NOT wishing to provide biometrics'. What does this mean? Which one should i go with?

4. In courier form there is a section 'In case of Non Serviceable Location, I agree to collect the above mentioned passport(s) from the below mentioned BLUE DART (HAL) Location:' but no location has been mentioned. What should be done? Do i have to find and fill nearest bluedart center myself?
5. In courier form there are two sections 'SO Done By/Date' and DO Done By/Date'. What shall be filled here?

Regarding the first point, if someone is not living at the place which is mentioned in their passport, how can they add it in the form? Isn’t their any other way to add our current address?

Regarding the first point, if someone is not living at the place which is mentioned in their passport, how can they add it in the form? Isn’t their any other way to add our current address?

The passport would be returned to the address you write in the courier form. I think this one is just for matching with passport address. You must have written your old address during other processes such as visa filing, college/uni application right? Like those documents this must match to your passport address too i guess.

Having a doubt on consent form. Can we leave the 3 and 4 page. Biometrics has already been given on 2019.

I've left it blank. There are a couple of other threads here too where people have said that they've asked the support team about it and the support team had responded them to leave it blank.

Can anyone pls help me with the query. In the VFS consent form it says name should be printed. Does that mean it should be typed and not written by pen? How do we type in pdf document.

In case of non-serviceable address:
1. For VFS consent form - In my case the Address is not serviceable - should I still mention the same address and asterisk it below mentioning non-serviceable location - mention below blue dart office address.
2. For the courier form - there is no need to fill the section 2(for non-serviceable address) - as I will be filling section 3 (blue dart location address ), please confirm.

I have used my current address in all the other places while applying for visa but my passport has a different address. Will mentioning the address on passport not create any confusions? Pls help
