Help Needed for APEGS #computer science self assessment form

Dear members,

We are filing for SINP nomination. We are trying to get a license from APEGS for : Engineer-in-Training - Intl grad.
This requires a self assessment to be done course by course evaluation report. We have a sample for Mechanical engineering self assessment report however my four degree is in Computer Science Engineering.

If anyone can please help me in sharing a similar domain report, i would be extremely grateful.
