Please i do have a question regarding the IMM 5604 form,(Declaration from non-accompanying parent/guardian for minors immigrating to Canada)
Principal is me and i have 16 years son.
I have sole Parental Authority and Custodian for my son.
and my EX husband never touch with my son and me after divorced in 2014. also never received the Child Support from him at all.
Today, I have a inquires from CIC for finger print clearance check and IMM5604.
I need to be complete the signed from my ex husband for approval my son's immigration.
how can this form be completed and by whom please? or do i make a explanation letter for this to CIC?
