Help in filling FBI PCC form

Hi All,
I am filling FBI PCC application form. I have couple of basic questions -
- In the from, it asks for "Reason for Request", what should i choose - Personal review / Challenge information on your record/ Adoption of a child in US/ International adoption / Live, work, or travel in a foreign country/ Other

If i choose other, then i dont have space to mention that it is required for PR

- Also do i need to attach cover letter with the form, finger prints.
