Help] IMM5604 form replacement

Hi ALL,

i got the request mail from CIC

Inquire of Fingerprint and IMM5604

> IMM 5604 Declaration: Please submit a completed Declaration from Non-Accompanying Parent/Guardian for Minors Immigrating to Canada Form (IMM5604) for. Note that a photocopy of a valid and legible identity document of the Non-Accompany Parent/Guardian must be included. This must be received at this office by: 2018/03/12.

but, i have no contact with my child's father actually he never paid child support and never visit my son.
do you think it is available to send other proof as like ( my ex husband gave me Parent authority and custody when we do divorce agreement)

Please give me advise for this issue.
