HELP! IMM0008 (11-2015)

Hello,
I've filled out the form Imm0008 form using Adobe Acrobat reader multiple times now, and after I validate the document it produces two additional pages that are more-or-less blank with small letters and numbers ( imm0008_11-2014_1 ) And it also says that the document now contains barcodes, but there are no visible barcodes.

Can anybody provide any insight into this?

I had the same problem. I don't know the reason but when I took the print out at my friends place who has a different computer and different printer, it worked, the bar codes appeared ! So try somewhere else.
