Help- IMM 5476 required in checklist

Hello experts,

In the ITA that i received, after i filled up the questionaire i got the checklist that also requires IMM 5476 as mandatory form.

I am confused- My wife and I are applying. She has not got this form under her checklist but only me.

So what should I do now?......Attach a letter stating I dont need a representative??? Or fill the form and assign my wife as my representative ( that I dont want to do as I take care of the application)

Please suggest your opinion. Thank you!

I'm guessing in your EE profile you selected yes to having a representative. You could sign it and say your wife is a representative. I *think* it just means that she can discuss your application with CIC if needed without you.

But I have not included anywhere that I need the rep in EE profile.

A quick q- if I include my wife as my rep and if the address on IMM 5476 is same as our mailing address then will that be an issue?

I'm genuinely not sure. I doubt it will be a huge problem. I think it is one of there files that is in reference to policy of who they can discuss your account with so I doubt it will have anything to do with your points or eligibility.
