Help! Help! Flagpole questions

Good day.. I am from Brazil, currently in Canada holding a multiple-entry visa that expires in 2017.
I got a job offer while visiting with a positive LMO as a Live-in-Caregiver (I am a physiotherapist in Brazil). I had done my medical exam already.

Which of the following situations apply to me (knowing, of course, that I rather not leave Canada)

-apply in Brazil for the work permit.
-apply online and then flagpole to activate my work visa.
-apply at the boarder as I flagpole.

You can't apply at the border. Not possible for LCP due to the medical requirement.

To the best of my knowledge, your application will go to Brazil regardless of whether you apply online or via mail. Your passport will also have to be sent to Brazil for visa stamping. And if an interview is required - it will be held there as well.

Do you think that I could send my application to the USA instead?
I have a multiple-entry visa of both (Canada and USA)?
