Hi everyone,
For my PR application, on July 28, CIC sent a request letter asking me for "Additional Family Info IMM5406". So I filled out, uploaded the pdf and submitted my application.
However, today, they sent me another request letter asking for the exact same IMM5406. I am not sure what is going on. They didn't tell me what's wrong with my uploaded form.
Letter from July 28: "Additional Family Info IMM5406: A separate and newly completed Additional Family Information form (IMM5406) bearing original signatures. This must be received at this office by: 2017/08/05"
Letter from today: > "Additional Family Info IMM5406: A separate and newly completed Additional Family Information form (IMM5406) bearing original signatures."
In my case, it is supposed to be really simple. I am the only child from my family, so i don't have any brothers or sisters. And I am single, so i don't have any spouse or common-law partner.
Here's the example of the form i filled out:

Could anyone please help me to figure out if i filled anything wrong with the form?
