Help for filling up immigration form

Hi everybody

I have questions about filling up the forms.

Firstly, I do not have my last name, so should I leave it unfilled or I should write N/A in this.

Secondly, should fill N/A in each and every space unfilled in the form.

I will really appreciate your help, guys.

dont leave any field blank. write N/A where you think it's not applicable.

i have written my name in one box...the other one is empty or i write N/A????????
