Hello everyone,

I am planning to apply for citizenship soon. I would like to know what should I provide as proof regarding the health visits to my local family physician?

Shall I request my family physician for all my visits records to the clinic?
I heard there is form that has to be sent to Ministry of health in order to obtain medical claim record from OHIP for citizenship application?

Would appreciate your feedback.

Evidence of doctor visits (medical record) is not part of the requirement for applying for citizenship. Evidence of doctor visits may be needed if IRCC demands more documents through residence questionnaire. So the answer is no.

Just follow the application checklist. Health records are not required.
