Once you are on PGWP (Post grad Work permit), you are NOT an international student anymore. Your status changes to that of a temporary foreign worker in Canada.
There are various insurance companies (big and small.. Canadian and international), which offer Annual health insurance coverage plans for temporary foreign workers in Canada. You need to do some research..and find the best rates and clearly understand the terms and conditions before purchasing a personal health insurance.

Some provinces like Alberta, Manitoba and Saskatchewan provide basic health insurance to temporary foreign workers:
