Health Card ID Proof

Hi all, I am applying for citizenship. I want to use Ontario health card as ID proof. However, I moved to Quebec province long ago, but Quebec health card doesn't have photo. In addition, Ontario health card isn't expired yet. Can I use that as ID proof or will it be a problem? Thanks in advance.

If your current residential and mailing addresses are in Quebec using an ON's health card may not be a good idea in my opinion. If you have a second ID with a photo you don't need a health card to have your photo. If I were you I won't try to make things more complicated, just get a first ID with picture and use your QC's health card as a second ID. This is just my opinion.

You could use your passport as one of your IDs. For the second ID, you could use your PR card.

photocopies of two (2) pieces of personal identification.

Examples of identification you can use include:

  • a copy of the biographical page of your passport/travel document (as requested above and also can be one (1) of your pieces of personal identification)
  • permanent resident card (PR card)
  • driver’s licence
  • health insurance card
  • senior citizen identification card
  • age of majority card
  • foreign identity documents, such as a passport or government issued identification documents
PR card is a new addition to this list. It wasn't there before. In fact, it was explicitly mentioned that you can't use your PR card but now, they are permitting it.
