I'm a Canadian and my wife is a visitor having British passport. Applied for Spousal sponsorship and meanwhile if she get a baby, can we get free medical assistance or OHIP? thanks!

In general, no.

Did you apply inland or outland? Hopefully outland.

If applying for PR inland she can get OHIP after 1st stage approval (14 months) + 3 month waiting period = 17 months.

Or if applying outland she can get OHIP after getting full PR (around 8-10 months through London office) + 3 month waiting period = 11-13 months.

But if having a baby you can try to get services of a midwife which are free even for visitors with no OHIP, you would only pay for ultrasounds, blood tests and hospital fees if you deliver in a hospital.
