Hello All,

I am from India.

In My passport my surname is - ALIAS PARTH GOYAL
and My Given Name is - ANIRUDH GOYAL

And in all my academic records my Name is Anirudh Goyal.

So What Should I mention in Have you use any other name ?

Should I mention Anirudh as the Given Name and Goyal as the family name ?
