1. You need a TRV, Temporary Resident Visa the application and instructions are here: http://www.cic.gc.ca/english/visit/tourist.asp If you are not a green card holder.
2. If you are Permanent Resident of the US (Green Card Holder) then you only need to apply for an ETA to visit Canada by air, ETA is not required if you enter by land.
