Has XYZ ever been refused a visa......?

Hello, I am at the very last page of PR application and I have these 2 questions simultaneously-
1- Has XYZ been refused refugee status, or an immigrant or permanent resident visa (including a Certificat de sélection du Québec (CSQ) or application to the Provincial nominee Program) or visitor or temporary resident visa, to Canada or any other country?

2- Has XYZ ever been refused a visa or permit, denied entry or ordered to leave Canada or any other country?

In my case I was refused US Student F1 Visa once(got accepted in second attempt). In which questions should I say "Yes" and put the details?

Yes to both.

You can also submit a Schedule A upfront as it might be requested during the processing of your application for answering "yes" to a statutory question.

Thanks for your reply. What is a schedule A form? I am filing PR via express entry and did not see it as a requirement. Can you please elaborate? Thanks!
