Yes I am coming from the US, and I am using U-pack as my moving company. I really appreciate your information, it was very helpful. I am assuming that you are already in Canada, Am I right? I just had one more question for you. What province do you live in? If Ontario, how long did it take to have coverage by your health insurance?

Of course I can't guarantee what worked for me will also work for you, but ... I used general categories of stuff - check moving companies, some of them have downloadable forms. If you're using a moving company, they may have their own form. It helps to go room by room. Of course I had to make up some of my own categories, esp. for relatively high value items that didn't fit in other categories. It was fine, no one ever even looked at my list, but perhaps that's just the luck of the draw.
I did list fairly detailed contents on each box, but that was overkill (of course, it helped me find stuff afterwards). Get the recommendation of what ever company you're working with, if you don't have one yet, just find a major company that goes cross-border and has forms online. Use the same categories on your B4. (e.g. automotive supplies).
Be sure to check an up-to-date list of what you CANNOT ship across border - anything potentially explosive (that included all cleaners). When I moved, mattresses were blacklisted... (unless they were new and factory sealed).
