Going to bank for OOPNP, what should I tell them?

Hi Guys,
I am going to TD bank tomorrow to get documents required for showing proof of fund for OOPNP. I am going to get bank statements and I know I should get another paper to show my balance. what is this document called? should I also get any letter from them? if yes, what is the format of the letter?

You can call that as Bank Certification Letter. Watever, you call, it should include the following info.

Name of account holder/s
Address of account holder
Type of Account
Account creation date
Bank Name and Address
Average balance in your account for the past 1 year and 6 months.

Thanks for your reply. So I went to the bank and got customer account confirmation, personal assessment and last 6 months bank statement. Do you think this is enough?
