GMCS notes - new consent form

Who signs the new representative line on the consent form in section C? I tried calling CIC today to no avail.

The ATIP department is the one who processes the GMCS notes, so they would be the ones to ask.

I filled this out and it was kicked back because I did it wrong. I filled out the notes for myself (the applicant) and wanted to be able to see my sponsor's sections....which is why i filled out the form. ATIP said in my case to list my Sponsor in section A and have him sign....then have my name (applicant) listed in section C.
