Generic application form

hi..do you need to fill up the generic application form to the province who'll be nominating you? or you just need that after they nominated you?
i just got confused because i don't know the UCI.. what should i suppose to write to that?

if you were not sent UCI (unique client identification), you can leave it blank..it will be given to you after you submitted your application in your VO, you can see that on the upper right corner or your medical form..and in AOR, i think
