I applied for GCMS note through gcmsonline.com. I received a form where mentioned "Designated representatives information" with other person. Should I make blank or fill up the data remain as usual pre-filled. Please help

In the instructions they tell you what to fill. Don't mess with the rep information. Just fill your name and information.
