What date determines when the 10 year look back period is?

For example:

If I send my materials to CIO in March 2009, the 10 year period would be March 1999 to February 2000

Now, let's imagine my last month of 6 years of full-time work in one of the 38 jobs was February 2000.

Now let's imagine I get an assessment letter in May 2009 asking me to furnish additional documentation

As part of this documentation, I will need to submit the same forms again.
Are these copies of what I submitted (with the March date) OR
Do I fill them out again and sign them with a May date?
If so, what happens to my 10 year look-back period? Did I lose 2 months?

Ideas? Any more specific info on the 10 year look back?

You just need 4 years of experience to claim 21 point dont worry about the 2 months.
I am jobless since last 1 year but i have more than 4 years experience

One question
In the guide its been said
sign and date all forms
what does it mean
shall one has to sign each and every page or just the last one?
If yes then on form Application for permenant residence theres no place to sign then?
Does thsi mean that one has to after filling the forms and making copies shall have to sign each page
Any suggestions
