There are some help required by intelligent persons while filling the form.I know that package will be change but maximum details will be same as old.
1) Full Names(s) (exactly as shown your passport or travel documents) - What to write (only surname or full name).
2) Have you ever used any other name (e.g nickname, maiden name, alias etc)? - About wife (surname is same as mine but differ in father's name) will have to tick yes / no.
3) schedule A background/declaration form : point no 8, what to write in status in country.
4) schedule A background/declaration form : point no 7 education : what to write in field of study for 10th and 12th (india)
please give as reply at the earliest.
