from which city I apply for PCC? Which address should I use?

I'm from Punjab, India. So answer according to the rules and laws of India specifically Punjab state.

I live and work in City-A but all my bachelor degree certificates, national ID card (Aadhar Card), voter card, passport, driving license, every other certificate uses the address of City-B. I live in a rented house & the owner of our house lives in a different city i.e. City-C. I don't want to involve the house owner bcoz of some personal reasons and we never had any agreement but paid house rent every month. My neighbors knows me very well and can give testimony that I'm living here and was never involved in any criminal activity. Now the thing I'm confused about -

1. From which city should I apply for PCC? City-A or City-B or both because I spend weekdays at City-A & weekends at City-B with family
2. If I've to apply from City-A then, what kind of documents do I have to show to the police officer? and from City-B what docs do I hate to show?
3. Do I have to show some kind of notary attested affidavit about the place where I lived with testimony of my neighbors.
4. Do I've to involve my house owners? I don't have any rent agreement signed with them but paid rent every month.

You mentioned you only live and work in city A but your docs. have the address of city B. so it has to be from city B. If you were living and working in a different country thn you would have needed two different PCC one for each. but since the country is same, so u need only 1 pcc and that is from city B

Why complicate it so much when you can just get one done from the passport office which basically covers the entire country?

I've been reading forums and got confused thats why. Call it my naivety. :p
So if I apply from any passport office it'll be enough. Thanks.
Do I've to take any person to the police station from area (permanent address) for any kind of testimony or guarantee?

Nope. If the passport office decides you need a police verification, they will tell you at that time or else they will issue the PCC on the spot. If you need police verification then someone from your area's police station would visit you for further processing. At this time they would need signature of a neighbor.
