if you go to an authorized fingerprinting agency, they will be having the fingerprinting card which meets the FBI requirements.

you can print the card and teh cover letter from there.

The Printing card is provided by finger printing agencies. But you can also take finger prints on form printed from on normal A4 size paper printed from following link
