Hi all,

In IMM5669E form under 8 and 12, we have From and To dates. I am filling the form electronically and since in one of the addresses I am still residing and in one of the companies I am still working - I am not sure what should I give in To Date. It does not accept characters as I want to enter "Till Date". Please help

Write the date that you will be likely to finish and sign the forms. Or even today's date. You will most likely be hand writing a few other letters, so you can add a side note that you are still employed and living at the work place/address.

If your sending a letter of employment or a lease/rental agreement they can put two and two together, but sometimes I think its best not to leave any loose ends.

Sign of a certain "end date" or "the date you will sign" is acceptable
 
I should add this note in a separate sheet or in the same sheet.

Also, there are various sections in other forms where there is not enough space to fill information, in such cases it says to use separate sheet of paper. Does it mean we can use a blank sheet of paper with our sign on it or should it be another copy of the same form?

You can use blank paper
