Hi,
Some forms have questions where you need additional space on additional paper. Where do you put those pages? Do you put them on top of the forms, underneath the page, or behind the last page.

Thanks, I'm trying to organize it now.

We put them all behind the last page of the form.

Make sure at the top of the additional pages you put the form number, name, DOB and UCI if you have one. So if they become separated it will be easy to figure out where they go :)
