Forms IMM 5669 and IMM 5406 and IMM 0008DEP

I need help. Guides aren't too clear and document checklist confusing me. I am pretty sure I'm asking a repeated question...sorry.

For IMM 5669 - Schedule A Background/Declaration. Is this filled by my husband who will be the Principal Applicant (sponsored)? There are 2 boxes to tick as either Principal applicant or Spouse, common-law, dependant children over 18

For IMM 5406 - Additional Family Information. My 2 children and I are canadian citizens so just to confirm, only my husband the principal applicant will need to fill this? Or will he also need to add me as spouse under Section A? Under section A there's also line with mother and father....does he need to fill this out? Under the other Sections it states Children and Brother and Sisters. I am confused. What sections of this form will need to be filled?

For IMM 0008DEP - Additional Dependants/Declaration. On the document checklist it states this form to be completed by the principal applicant, if you have more than 5 family members. What does this mean? I am trying to sponsor my husband (outland) and my kids and I are already canadians living in australia. Do we need to fill this form? Or is this a non-applicable for us?

Hopefully i made sense.

If anyone can shed some light on this... would greatly appreciate it

hi there...these forms are for the applicant or dependents who are being sponsored and are over 18 - as Canadians (or being under 18), your children and you (who I'm assuming is the sponsor) are not required to fill these out. If your husband is the only applicant then he's the only one that needs to fill them out. So, IMM 5669 and IMM 5406 are only for your husband or, if applicable, his dependants that are over 18 that you would be sponsoring into Canada as well. But it doesn't sound like he has any.

Form IMM 5406 - Your husband has to list ALL his immediate family members - brothers, sisters, children (including step) and parents - and provide all of their information - marital status and address, place of birth etc. You need to be listed as his spouse and your children need to be listed as his step-children.

For IMM0008DEP - does your husband have more than 5 dependants? If not this doesn't apply to you. You can either include the form (and any others that you don't need to fill out) in your package with N/A written on it, or write N/A on the document checklist. I just wrote N/A on the checklist and got AOR no problem.

Definitely cleared a few things up for me.
So for IMM0008DEP, my husbands so called dependants are our 2 kids but they are both canadian citizens so we can just do an N/A on this form eh?

Also, another question...on Form IMM0008 - Generic Application Form, on question #3 it asks How many family members, including you , are in this application?
As I am sponsoring only my husband, does he only put 1 on this question?

You don't even need to include IMM0008DEP. Just write N/A for it on the checklist.

Yes, your husband puts 1 as he's the only person being sponsored.
