FORMAT OF COMBINED PDF FOR STUDY VISA

Guys, I have a doubt that when uploading academic transcripts should I attach both the front and back page or only front page of the transcript? Should I put the document name as a heading over scanned copies for each document in a combined PDF?

Example- "Recent academic transcript"(Single PDF)
-Transcript(Heading)
scanned document
-Semester grade sheet(Heading)
scanned document
-Degree certificate(Heading)
scanned document

Scan as is.
Leave out blank pages with no page number.
Do not alter the original documents in any way. Do not write anything on them.
If there are multiple documents, create a table of contents as page 1.

In your case, just scan as is. No annotation/headings required.
