Question 12 of the said form requires to list the addresses where I lived since my 18th birthday OR the past 10 years, whichever is recent.

Per instruction I am allowed to list only the past 10 years. By that i may not submit police clearance from Saudi Arabia and Qatar where I worked from 1989 to October 2004 combined, which is already outside the 10 year period. How will CIC know that I stayed on those places?

Per instructions as you put it, CIC only needs to know the last 10 years, if of course, this comes first. And trust me if they want to dig deep, they can and they will.
So you are better off with following the instructions. I had the same situation and I listed the last ten years. And before that I was all in my home country but in different cities.

You are mixing things up. Regarding police checks, it clearly states on the document checklist:

You must get a police certificate from each country or territory where you have lived for six months in a row since the age of 18.

The 10 year period does not apply to police checks.
