Form 5540 question 7 - self employed - address? income proof required?

HEre's one: I'm currently self-employed and question 7 is of course asking for my employer and address etc... Shall I just give my home (I work from)? Also, is CIC looking for my exact annual income with invoices & proof? Because there is no income requirement for this sponsorship.
