Forgot to include consent form for ATIP/GCMS

I'm a sponsor and I ordered GCMS notes on my wife's behalf a few days ago (she's the PA), but I didn't know at the time that I needed to include a consent form signed by her. Is there any way I can get it signed and send it over to them before they ask? I just want to avoid delay.

did you get a confirmation email ???

you better write back to them with details like receipt number and explaining the situation
my notes were delayed when i wrote to them they responded on that email

alright, I'll try doing that. as for the consent form itself, will she have to mail in a signed paper copy, or print/sign/scan and email, or just email the filled out pdf?
