Forgot to disclose Canada visa refusal

Hello I submitted my pr application around 1 month ago. Then today after spoke to my parents I realized I didn't disclose that I was rejected a Canada visa 20 years ago when I applied as a minor/dependent. I didn't submit biometric then. Should I raise a web form and add LOE? Do I need to add IMM5669 form ?
