That`s all, you don`t need to include a letter unless you want to explain your relationship in more details and you include the pregnancy as well.

OUTLAND Form: IMM5490E - there is not question about it
INLAND Form: IMM5285E - question number 8 - Are you or your sponsor pregnant?
However on IMM5490(For outlanders) there is a space where you should explain the development of your relationship, you should use that space to mention/explain your pregnancy.
Please correct me if I am wrong.
