For New Born baby how many document need

Can you tell me please as per cic rule for new born baby witch form and how many document need to send?

Hi friends;
Me in the same boat. My understanding is that we have to resend following forms:
1. Application for Permanent Residence Form.
2. Additional Family Information forms for myself & family.

Additional Docs:
1. Birth Certificate of new baby (notarized).
2. Copy of Passport of new baby (notarized).
3. 04 Pics as per checklist.
4. DD of 150 CAD.


I will also send a case specific inquiry to CHC on subject. Plz let me know if u have any comments.

Please include the notarized "Medical" Birth Certificate of the new born and also the notarized "Jonmo nibondhdhon" certificate for the Bangladeshi born baby. the other things are mentioned above.

Dear abrargujjar and zafrulbd

Below is the detail of documents or actions that you need to take in order to comply fully with CHC requirements for new born;

1. NEW BORN Birth certificates (hospital and ministry of health). No need of notarization. Only copies can serve the purpose.
2. Passport of new born (and visa page if residing outside homeland)
3. Five photographs as per specification,
4. Additional family information forms (update) and signed in original
5. Demand draft of CAD 150/- (in the name of RECEIVER GENERAL FOR CANADA)
6. Information on height of new born at the time of sending forms
