1- Driver's license : Should I take picture from my mobile of both back and front and upload it - is this fine ?

A scanner is generally a better tool for this purpose. You can set the DPI and avoid common quality issues. If you don't have access to one you can go to Staples or some other place.

That being said, you can for sure create quality scans using your phone. Just make sure it's not shaky, everything is readable and no reflections, particularly for cards. Format it to match the upload requirements.

For any sort of identification card, you need both front and back.
