First time PR FSW form => Please confirm

Hi ,

I am on H1 Visa and want to apply for a PR. Currently i do not have any job offer. I am in the IT industry and would like to confirm my FSW form before i submit it. This would be helpful to all other persons in same category ( i guess many others)

B16, B17 and B18 - where in Canada did you become PR, etc << Leave blank ? >>
C21: Travelled outside of Canada << Leave blank ? >>
D22: Outside of Canada for more than 1095 days << Leave blank ? >>
F24: Lost << Leave blank ? >>

Form IMM 5455
How should i apply the photo. It says do not staple , glue or use paper clip.

Copy of passport
Copy of Provincial Driving / PR /Landing / Income Tax << DONT HAVE ANYTHING OUT OF THESE OPTIONS ??] >>
2 photos with Canadian Specs (one on imm 5455 and one in envelope)

Well you won't have much luck, since it appears that you are looking at the wrong forms. Those forms are for applying for citizenship, not Permanent Residence. See: http://www.cic.gc.ca/english/information/applications/skilled.asp You also have to have 1 years experience in the List of "38" to be eligible see: http://www.cic.gc.ca/english/department/media/backgrounders/2008/2008-11-28a.asp
