Finishing paperwork and have some questions

So my wife was wondering about some questions on our pr paperwork and she wanted me to come ask them on here.

First: She was wondering what to put down on any of the questions wanting to know her last name. She hasn't had time to get her passport changed, but she uses my last name in her signature. Which should she use?

Second: My wife now lives with me, should I use my address for the question (address where the person you are sponsoring lives) or should I use her old address in the states?

First: She should use the name that she has her legal passport and identification in. She should also sign that until its changed legally.

Second: She needs to use the address where she will be, as there will be mail being sent there and if it is her old address then she won't be able to get it.
